2. Be reminded that during this pandemic time, NHSDI Davao is strictly observing social distancing, surgical mask use, face shield use and proper handwashing. You are expected to follow these guidelines once you enter our premises.

3. You will be asked to fill up the ‘symptoms checklist form’ as required by DOH and DTI. Please be honest in filling up this form as this can be a legal matter which concerns public safety. Please do not come for medical check up if you have any symptoms like fever, cough, headache, lbm, sore throat, colds and the likes. If you are not feeling well please stay home and call the designated Numbers of Covid Centers.

4. Let us practice the guidelines set by WHO/DOH to stop the transmission of Covid 19. Together we can win this battle.

The following MEDICAL EXAMINATION PACKAGE FEES will be in effect in all Clinics of the NATIONWIDE HEALTH SYSTEMS GROUP starting 1 August 2022.




BEFORE MAKING AN APPOINTMENT AND PROCESSING PAYMENT, MAKE SURE YOU SHOULD CONTINUE YOUR SCHEDULE BECAUSE THE REFUND PROCESS WILL TAKE 3 to 5 MONTHS AND THAT WILL BE SEND TO MAIN BRANCH AND THEY WILL CHECK IF THE PAYMENT HAS BEEN DEPOSITED.

REMINDERS:
 Ø Only applicants will be allowed to enter the clinic except for elderly and children and those who need chaperone.
 Ø Please be honest in answering the DTI CHECKLIST. False declaration is punishable law under RA 11332.
 Ø MASK AND FACESHIELD MUST BE USED PROPERLY ALL THE TIME
 Ø SOCIAL DISTANCING OF 6 FEET MUST BE OBSERVED.

SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination): You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.

THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION: The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.

    - Vaccination Record (must contain the following details):
    • personal copy (e.g. baby book)or copy of medical chart indicating received vaccines
    • complete date of receipt of vaccination
    • must include signature (license number, if applicable) of doctor or health worker who administered vaccine

    1. Before coming for your medical examination, you must lodge your visa application, either with the Australian Embassy or via eVisa systems

    3. FOR PREGNANT Applicants please bring OB_Gyne consent prior to xray.

    • Permanent Visa Applicants 2 to 10 years old are required to undertake either a Tuberculin Skin Test (TST) OR a TB Blood Test (Interferon Gamma Release Assay or IGRA).
    • Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is between 48 and 72 hours from TST placement.
    • To avoid TST from becoming invalidated, please be at the clinic at least 1 hour before the schedule written on your TST return slip.
    • TB Blood Test (Interferon Gamma Release Assay or IGRA) will only require 1 visit if results are negative.
    • Chest X-ray will be required for children 10 years old and below if they have a positive TST/IGRA result or have exposure to TB cases or a history of TB disease.

    Starting January 31, 2022, New Zealand TOURIST visa applicants are required to submit their visa application online first prior to their Immigration Medical Examination (IME). Please visit INZ website https://www.immigration.govt.nz/ for more information.
