REMINDERS FOR THE APPOINTMENT

1. The Appointment Confirmation will be sent by e-mail. Please present a digital or printed copy to the Screening Officer at the 2nd floor entryway near the spiral staircase.
2. Laboratory tests DO NOT require fasting. Applicants are advised to take their meals prior to their medical examination as food is not allowed in the premises.
3. Please be at the Clinic 30 minutes before your scheduled appointment. Upon arriving at City Hub building, please proceed to the 2nd floor entryway near the spiral staircase first for initial screening.
4. Walk-in applicants may be accommodated after all appointments for the day have been served, or when there are vacant time slots.
5. One (1) chaperone per family will be allowed inside the clinic. We therefore strongly discourage clients from being accompanied aside from the allowable chaperone. For senior citizens, PWDs or minors who will need to be accompanied, please inform us beforehand so arrangements can be made.
6. CASH payments in PHILIPPINE PESOS or Advanced over the counter CASH deposit to our BDO account are accepted.

WHAT TO BRING ON THE DAY OF YOUR APPOINTMENT

1. NHS Baguio Appointment Confirmation letter
2. Referral letter from the Australian Embassy which contains your HAP ID / TRN
3. Any one (1) of the following acceptable forms of identity documents:
   • Current valid passport
   • Passport which has expired within two (2) years of the health examination date
   • Current valid emergency travel document
   • Current valid temporary travel document
   • United Nations High Commissioner for Refugees (UNHCR) recognized / recommended document
   Non-migrating dependents applying for permanent visas who do not have a passport or have already submitted their passports to the Australian Embassy at the time of their appointment will be allowed to proceed with their IME provided they present any two (2) of the following government-issued identification documents:
   • Unified Multi-Purpose ID
   • Bureau of Internal Revenue (BIR) ID (laminated with photo)
   • Voter's ID (laminated with photo)
   • Social Security System (SSS ID)
   • Professional Regulations Commission (PRC) ID
   • Driver's License
   • Postal ID
   • Birth certificate issued by the Philippine Statistics Authority (PSA) AND at least one form of identification with photo (e.g., school ID) for children between 0-15 years old. (Note: Birth certificates issued by the Local Civil Registrar (LCR) will not be accepted. Applicants born in a country other than the Philippines must present a government-issued Birth Certificate from the country of birth.)
4. COVID vaccine card / record
5. Immunization records, if available, for children between 0 to 15 years old
6. Medical Certificate/s for a previously treated or currently managed clinically significant illness which should contain the following information:
   • Duration and date of treatment
   • Medical treatments / medications given or surgical procedures done
   • Complete final diagnosis
   • Further management needs recommended
   • Prognosis
7. For applicants with a history of Pulmonary or Extra-Pulmonary TB treatment,
   • Old chest x-ray films not less than 6 months old prior to the medicalexamination
   • Medical Certificate/s containing the information required in no. 6 above
   • DOTS record
8. Letter from the attending Obstetrician-Gynecologist of a pregnant applicant allowing her to undergo a chest x-ray (CXR)
9. Prescription spectacles / eyeglasses or contact lenses
10. Others
    • Surgical mask, which is required to be worn within the Clinic premises
    • Valid ID, which is required to be shown at the 2nd floor entryway near the spiralstaircase of the City Hub Building
    • Senior Citizen / PWD ID, which is required to avail of the applicable discount
    • Black or blue ballpen

NOTES ON TB SCREENING TESTS (IGRA)

• TB Screening tests (TST / IGRA) will be required for Permanent Visa applicants aged 2 to 10 years old, as well as for Health Care Workers or Health Care Students.
  • Interferon Gamma Release Assay (IGRA) will only require 1 visit but will be referred at St Luke's Medical Extension Clinic for the test
  • Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is 72 hours from TST placement. Schedule for TST test is as follow:

• A chest x-ray (CXR) will be required for children 10 years old and below whose IGRA tests yield positive results or have exposure to TB cases or a history of the disease.
