ANNOUNCEMENT
The Ministry of Health, Labour and Welfare (MHLW) of the Government of Japan has designated NATIONWIDE HEALTH SYSTEMS BAGUIO, INC. as a PANEL CLINIC for the Japan Pre-Entry Tuberculosis Screening (JPETS) Programme.

IMMIGRATION MEDICAL EXAMINATION (IME) PACKAGE FEES

The abovementioned fees include the following:
- cost of TB screening, additional chest x-ray views, and sputum examinations, when required
- cost of chest x-rays (PA and lateral views) for applicants aged 0-4 years old with positive IGRA results.
They do not, however, cover the cost of treatment of active tuberculosis (TB) or latent tuberculosis infection (LTBI).
REMINDERS FOR THE APPOINTMENT
- The Appointment Confirmation will be sent by e-mail. Please present a digital or printed copy to the Screening Officer at the 2nd floor entryway near the spiralstaircase.
- Laboratory tests DO NOT require fasting. Applicants are advised to take their meals prior to their medical examination as food is not allowed in the premises.
- Please be at the Clinic 30 minutes before your scheduled appointment. Upon arriving at City Hub building, please proceed to the 2nd floor entryway near the spiral staircase first for initial screening.
- Walk-in applicants may be accommodated after all appointments for the day have been served, or when there are vacant time slots.
- One (1) chaperone per family will be allowed inside the clinic. We therefore strongly discourage clients from being accompanied aside from the allowable chaperone. For senior citizens, PWDs or minors who will need to be accompanied, please inform us beforehand so arrangements can be made.
- CASH payments in PHILIPPINE PESOS or Advanced over the counter CASH deposit to our BDO account are accepted.
- If you are undergoing pulmonary or extra-pulmonary TB treatment, we encourage you to complete the treatment before scheduling your appointment for JPETS.
WHAT TO BRING ON THE DAY OF YOUR APPOINTMENT
- NHS Baguio Appointment Confirmation letter (digital or printed copy)
- Guarantee/Reference Letter from your sponsor/employer (if available)
- Valid Original Passport

For minors (17 yrs. old and below) who will not be accompanied by their parents
during the medical examination, please bring:
- Birth certificate
- Letter of authorization stating that the companion or guardian is authorized to sign on behalf of the parent/s.(Note: The companion or guardian must be knowledgeable about the minor’s medical history.)
- Copy of parent/s’s valid government-issued ID bearing a verifiable signature
- Valid government-issued ID of the companion or legal guardian bearing a verifiable signature

For applicants with a history of pulmonary or extra pulmonary TB treatment
- Old chest x-ray films not less than 6 months old prior to the medical examination

Medical Certificate/s containing the following information:
- Duration and date of treatment
- Medical treatments / medications given or surgical procedures done
- Complete final diagnosis
- Further management needs recommended
- Prognosis
- DOTS record

- Surgical mask, which is required to be worn within the Clinic premises
- Valid ID, which is required to be shown at the 2nd floor entryway near the spiral staircase of the City Hub Building
- Senior Citizen / PWD ID, which is required to avail of the applicable discount

NOTES ON TB SCREENING TESTS (IGRA)

TB Screening tests (TST / IGRA) will be required for Permanent Visa applicants aged
2 to 10 years old, as well as for Health Care Workers or Health Care Students.
- Interferon Gamma Release Assay (IGRA) will only require 1 visit but will be referred at St Luke’s Medical Extension Clinic for the test

Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is 72
hours from TST placement. Schedule for TST test is as follow:
First Visit Second Visit Schedule 1 Wednesday Saturday Schedule 2 Friday Monday
- A chest x-ray (CXR) will be required for children 4 years old and below whose IGRA tests yield positive results

SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination): You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.
THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION: The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.
