WHAT TO BRING ON THE DAY OF YOUR APPOINTMENT
- NHS appointment confirmation letter (digital or printed copy)
- Valid original passport
- For minors (17 years old and below) who will not be accompanied by their parents during the medical examination
- Birth certificate
- Letter of authorization stating that the companion or guardian is authorized to sign on behalf of the parent/s (Note: The companion or guardian must be knowledgeable about the minor’s medical history.)
- Copy of parent/s’s valid government-issued ID bearing a verifiable signature
- Valid government-issued ID of the companion or legal guardian bearing a verifiable signature
- For applicants with a history of pulmonary or extra pulmonary TB treatment
- Chest x-ray films/images (preferably in a CD/DVD), if available, 6 months or older prior to the JPETS medical examination
- Medical records/TB DOTS records, if available
- We only accept cash payments.
REMINDER
If you are undergoing pulmonary or extra-pulmonary TB treatment, we encourage you to complete the treatment before scheduling your appointment for JPETS.

The abovementioned fees include the following:
- cost of TB screening, additional chest x-ray views, and sputum examinations, when required
- cost of chest x-rays (PA and lateral views) for applicants aged 0-4 years old with positive IGRA results.
They do not, however, cover the cost of treatment of active tuberculosis (TB) or latent tuberculosis infection (LTBI).
ALL APPLICANTS 18 years old and below must be accompanied by their parents or any legal guardian who has thorough knowledge of the applicant’s medical history.
All are required to wear SURGICAL FACE MASK within the clinic’s premises.
Companions will not be allowed inside the clinic, and we therefore strongly discourage clients from bringing one. (For elderly and PWDs who will need to be accompanied, please inform us beforehand so arrangements can be made.)
For your safety, all visa applicants will first proceed to our Screening Room at 24A (Second floor near the elevator) before entering the main clinic.
SPECIAL NOTE FOR WORKERS (Regarding the Visa-Related Medical Examination): You are here to undergo an embassy-required medical examination as part of your visa application. The examination and tests are done to ensure that no public health, public safety, or unreasonable demand is imposed on the host country.
THIS IS NOT a POEA-REQUIRED MEDICAL EXAMINATION: The VISA-RELATED MEDICAL EXAMINATION is different and not accepted by the POEA just as the POEA medical examination is not accepted by the embassy. The embassy’s medical examination is done to protect its citizens. The POEA medical examination is done to protect the OFW.

REMINDERS FOR THE APPOINTMENT
- The Appointment Confirmation will be sent by e-mail. Please present a digital or printed copy to the Screening Officer in Unit 24A.
- Laboratory tests DO NOT require fasting. Applicants are advised to take their meals prior to their medical examination as food is not allowed in the premises.
- Please be at the Clinic 30 minutes before your scheduled appointment. Before entering the main Clinic, please proceed to the NHS Aux Screening Room / Unit 24A (turn left when you exit the elevator on the 2/F) for initial screening and registration.
- Walk-in applicants may be accommodated after all appointments for the day have been served, or when there are vacant time slots.
- Companions are generally not allowed within the Clinic premises, and applicants are strongly discouraged from bringing one. For minors, elderly and PWD applicants who will need to be accompanied or require assistance, please advise the Clinic beforehand so the proper arrangements can be made.
- Only CASH payments in PHILIPPINE PESOS are accepted.
WHAT TO BRING ON THE DAY OF YOUR APPOINTMENT
- NHS Aux Appointment Confirmation letter
- Referral letter from the Australian Embassy which contains your HAP ID / TRN
- Any one (1) of the following acceptable forms of identity documents:
- Current valid passport
- Passport which has expired within two (2) years of the health examination date
- Current valid emergency travel document
- Current valid temporary travel document
- United Nations High Commissioner for Refugees (UNHCR) recognized / recommended document
Non-migrating dependents applying for permanent visas who do not have a passport or have already submitted their passports to the Australian Embassy at the time of their appointment will be allowed to proceed with their IME provided they present any two (2) of the following government-issued identification documents:
- Unified Multi-Purpose ID
- Bureau of Internal Revenue (BIR) ID (laminated with photo)
- Voter’s ID (laminated with photo)
- Social Security System (SSS ID)
- Professional Regulations Commission (PRC) ID
- Driver’s License
- Postal ID
- Birth certificate issued by the Philippine Statistics Authority (PSA) AND at least one form of identification with photo (e.g., school ID) for children between 0-15 years old. (Note: Birth certificates issued by the Local Civil Registrar (LCR) will not be accepted. Applicants born in a country other than the Philippines must present a government-issued Birth Certificate from the country of birth.)
- COVID vaccine card / record
- Immunization records, if available, for children between 0-15 years old
- Medical Certificate/s for a previously treated or currently managed clinically significant illness which should contain the following information:
- Duration and date of treatment
- Medical treatments / medications given or surgical procedures done
- Complete final diagnosis
- Further management needs recommended
- Prognosis
- For applicants with a history of Pulmonary or Extra-Pulmonary TB treatment,
- Old chest x-ray films not less than 6 months old prior to the medical examination
- Medical Certificate/s containing the information required in no. 3 above

- Letter from the attending Obstetrician-Gynecologist of a pregnant applicant allowing her to undergo a chest x-ray (CXR)

NOTES ON TB SCREENING TESTS (IGRA)
- TB Screening tests (IGRA) will be required for Permanent Visa applicants aged 2-10 years old, as well as for Health Care Workers or Health Care Students.
- The cost for TB Screening tests (IGRA) for Health Care Workers or Students is Php 6,500.00.
- TB screening tests (IGRA) are done on Mondays to Fridays only.
- A chest x-ray (CXR) will be required for children 10 years and below whose IGRA tests yield positive results or have exposure to TB cases or a history of the disease.

1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state.
2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
3. FOR PREGNANT APPLICANTS please bring OB-Gyne consent prior to Xray.
